Short-Term Rehab

Bridging the gap between hospital and home

Whether as a result of a trauma, disease, stroke, surgery or chronic condition, when mobility, function or cognitive abilities are impaired, rehabilitation is often the pathway to recovery, renewed independence and quality of life.

A unique understanding of senior care

  • Nurses
  • Therapists
  • Nutritional consultants
  • Mental health professionals

Further, we educate all staff members to be vigilant regarding changes in our cardiac patients’ condition, which has proven to be an effective step in reducing re-hospitalizations. For more information regarding our efforts to reduce re-hospitalizations.
